improve readconcern docs

      there's been a lot of free support questions. i would search mongodb-user for readconcern.

      some specific things:

      • it sounds like find().readConcern() has a default, but it doesn't when i try it in the 3.2.0 shell
      • it's a little confusing how to do count() in the shell with readconcern. the table say "document" on the left but then shows strings on the right. there is an example at the bottom, which kind of saves the day, but definitely tweakable.
      • we should say what the default is for enableReadConcern startup option (default is off)

      maybe other stuff too, fyi.

      we also should explain a bit perhaps on when / where you would use this.
