I noticed this bug because after running tests, I had users with usernames "test_username_4" and "test_username_40", a uniqueness validation on username, and the following was returning false
>> User.first(:conditions => { :username => "test_username_4" }).valid?
=> false
This cleans up that logic and makes it clearer that case_sensitive should be true in the default case.
