Currently the saslStart and saslContinue commands set ok=1.0 and code!=0 to indicate failure. This is inconsistent with other commands. Failure should be indicated with ok=0.0.

Message: SERVER-8100 Do not expect a code in sasl commands that return ok: 1.

Also, correctly report error messages when ok: 0 and code field is present.
