&lt;p&gt;&lt;tt&gt;Whenever&#160;FindOneAndReplace is used, there is a chance that the database does not reflect the replacement. I am not totally sure why the document that comes back reflects the correct state, but not in the actual database.&lt;/tt&gt;&lt;/p&gt;
