Log x-request-id in MCP wherever we have access to it

      In streamable http server the headers get passed down, so at least in the atlas mcp case there should be an x-request-id header. It would be good to log this with whatever we're logging so that we can find these logs with the rest of the request's logs.

      This is a "best-effort" / easy win ticket. If we don't have easy access to the header where we log, we can file follow-ups.
