document cursor lifecycle

      We should document the lifecycle / lifetime of a client cursor. Cursors may be created when a query is performed and destroyed by a kill cursors request or a timeout.

      In particular a cursor may also be killed when an index not used by the cursor is dropped, which may not be obvious behavior.
