When using the "Report a Problem" link via docs.mongodb.org, a Jira issue creation form is displayed in an iframe.

It's currently not clear that:

• all information shared will be public
• additional browser headers will be recorded (User-Agent, Location, Referrer, Screen Resolution)
• all fields (aside from Summary) are optional

Perhaps should also have a link to relevant privacy policy.
