Undefined now maps to undefined in shell, not null

      In SERVER-6102, we've changed the shell so that BSON's undefined type maps to Javascript's undefined value rather than mapping to Javascript's null value. Any documentation for the shell that says undefined maps to null should be updated to reflect this.
