The current cursor.skip() reference is almost entirely from 2012, and could do with some updating.

Observations (mostly from andrew.aldridge):

• Behavior: 'consider' doesn't really mean what we're trying to have it mean. Also: the skip and limit statements are much too long for the line and should be broken up to be more legible.

• The 'often expensive' paragraph's technical accuracy with regard to CPU vs. IO performance is perhaps suspect.

• The final paragraph is dense
