Auto-cancel a proposed tool call when tools are disabled

      When a tool call has been proposed, before it is confirmed, if a user disables tool calling, we should auto-cancel the tool call.

      The assistant should also send a message that the tool call has been cancelled because of the change. 

      For example: "The pending operation was cancelled because database tools were disabled."
