This happened with Gaurab when he left his machine on idle and his connection died off. Post connecting - Gaurab ended up having new insights alongside the old stale insights where the old insights. Old insights were pointing No Collection / No Database reference for the query and at the same time the new insights were pointing some other insights.

Fix discussed on call was to clean the inspections flow on every successful connection.

As a sidenote: Please also check the AbstractInspectionsBridge - I think we are doing something similar to the suggestion already but worth checking.
