Refresh the IP resolved for hosts frequently enough to pick up dns changes for direct and mongos connections

      IP address resolution is cached in ServerAddress, and refreshed regularly by ReplicaSetStatus as part of background processing of the replica set status. But it's not updated at all for direct or mongos HA connections, so a restart is required to pick up DNS changes.
