Now that we support the exhaustAllowed bit for commands which support exhaust behavior, changes are required at the transport level to support the feature. Specifically, command needs to support an exhaustAllowed option, and the Connection needs to be able to track responseTo/requestId of synthetic {{getMore}}s and use the same callback for new batches.
