New Replica Set connections hang when secondaries are creating indexes (node native driver)

      We are seeing calls to MongoClient hang indefinitely when a secondary is creating an index.

      The connectionTimeout seems to have no effect (we still hang). Setting a socketTimeout results in the error: "Could not locate any valid servers in initial seed list"

      It looks like this could be related to https://jira.mongodb.org/browse/NODE-50
