Reorganize documentation as a single book

      Our documentation is currently structured as a set of books, which dates back to the (very) early time when the bson and mongodb extensions were separate entities.

      For consistency with other documentation, we should restructure our documentation as a single book. This will also overcome various challenges with organizing our content (e.g. PHPC-1246).
