Remove single write methods, WriteErrorException, and WriteConcernException

      These convenience methods can be introduced in a minor version if there is significant need from users; however, PHPLIB does not need these and omitting them from 1.0.0 will allow us to reduce a bit of complexity around them (e.g. special exception classes, unwrapping BulkWriteExceptions).
