Cursor::isDead() returns true despite cursor being alive

      Since upgrading libmongoc to 1.4.0-dev (PHPC-629), the following tests have been failing:

      • tests/cursor/cursor-destruct-001.phpt
      • tests/cursor/cursor-isDead-001.phpt

      See this Travis CI build for details. This may be an upstream regression in mongoc_cursor_is_alive(), but further investigation is required.
