APM functions should reside in MongoDB\Driver\Monitoring namespace

      The addSubscriber() and removeSubscriber() functions currently reside in MongoDB\Monitoring, but the command event classes and the subscriber interface reside in MongoDB\Driver\Monitoring. These functions should be moved to MongoDB\Driver\Monitoring.
