Collection::createIndex() does not pass "comment" option to createIndexes command

      Collection::createIndex() does not extract the comment option when delegating to the createIndexes() method. Instead, the option is left within the index specification document.

      This issue dates back to the original implementation of the comment option in PHPLIB-749.
