      Context

      Add the actual OpenTelemetry span support on top of the consolidated telemetry handling.

      Definition of done


      * Command spans are created per the DRIVERS-719 spec (github.com/mongodb/specifications/blob/master/source/open-telemetry/open-telemetry.md).
      * Full implementation plan: gist.

      Pitfalls


      * Build on the existing consolidated telemetry API rather than re-instrumenting individual call sites.
      * Keep the OpenTelemetry dependency fully optional.
      * Operation- and transaction-level span nesting are out of scope for this ticket.
