Mongo::Error::OperationFailure (not authorized on [db] to execute command....(13))

      MongoDB 3.2.1 with wiredtiger engine on Compose.io

      Our production server on Heroku is sporadically getting 

      Mongo::Error::OperationFailure (not authorized on [db] to execute command....(13))

      It happens just randomly. I am using all default options in mongoid.yml.

      It's affecting our production server, so any help would be greatly appreciated.
