Make SDAM debug logging to be less noisy

      A number of users complained about debug SDAM messages that look meaningless. For example:

D, [2017-06-19T12:14:30.254118 #36565] DEBUG -- : MONGODB | Server description for changed from 'secondary' to 'secondary'.
D, [2017-06-19T12:14:31.083621 #36565] DEBUG -- : MONGODB | Server description for changed from 'secondary' to 'secondary'.
D, [2017-06-19T12:14:41.139673 #36565] DEBUG -- : MONGODB | Server description for changed from 'secondary' to 'secondary'.

      It should not be necessary to log messages like these if there wasn't any change in the server's status.
