SERVER-71755 introduced a "two-phase protocol" for notifying the commit of a createDatabase command as a way to compensate the limited serialization properties of the operation.

Later design decisions eventually discarded the adoption of such a protocol, making most of the code supporting the generation of createDatabase[prepare|commit|abort] useless. Related source code can be safely removed.
