Rename host_adapter to host_connector

      We collectively decided to rename the host_adapter directory and namespace to host_connector. Since we've formalized the definition of adapter to mean an implementation of an API interface on the callee side, we need a term to encompass handle types and adapter types.

      In summary: adapters + handles = connector layer
