Add a field to the FCV document explicitly indicating at which phase the execution of setFCV is.

In this way we can make adding/removing phases flexible, instead of the current rigid design where the progress of setFCV must be inferred from the combination of the version, targetVersion, previousVersion and isCleaningServerMetadata fields.
