For Primary-Driven Index Builds, resuming happens on Step-Up, not Startup. Nevertheless, in the resume code paths, IndexBuildOptions.applicationMode == ApplicationMode::kStartupRepair is required such that the the index build actually resumes, and doesn't redo setup work. This should be renamed something to indicate that this is really a flag indicating resuming, regardless of whether it happens at startup or step-up.
