Generate and persist ShardRef during AddShard

      In this ticket, the addShard coordinator should (if the feature flag is enabled) generate a UUID to be used as the shardRef and persist it in config.shards and the shard identity. This should also use the shardRef in config.placementHistory and config.databases if this is a promotion to sharded.
