Remove resharding refresh code paths after 9.0 is last LTS

      In 9.1+, the coordinator will always use direct commands to communicate with shards rather than use refreshes. We should remove the refresh code paths because it is dead code and adds extra overhead that someone has to think about when they work on resharding.
