Remove handling of multikey piggypacked on insert side write entries in index builds

      After SERVER-135300, PDIB always puts multikey side writes in their own "m" entries rather than piggybacking them on corresponding "i" entries. Thus, once sufficient time has passed, we can remove the handling for the old format.
