Multi-threaded index creation

      In addition to building indexes in parallel (as documented in SERVER-12309), it would be useful to have multi-threaded index builds.

      In the scenario where there is one large index, building indexes in parallel won't help. The idea is to use all CPU threads on the system to speed up the index creation.
