improve debug heartbeat log messages

      We increased the verbosity of replication tests in SERVER-27908. The additional logging has been very helpful but also highlighted the need for clearer log messages with respects to heartbeat logging (especially to an operator who's not particularly familiar with the replication subsystem).

      This ticket describes the work to review the current debug log messages for heartbeats and to propose refinements.
