Improve the error message when unable to reach any nodes in a replica set

      The following error:

      "No primary detected for set Cluster0-shard-0 2017-06-14T00:00:00.000-0800 I NETWORK [ReplicaSetMonitor-TaskExecutor-0]
      All nodes for set Cluster0-shard-0 are down."

      should say instead print something more like the following:

      "Unable to reach primary for set Cluster0-shard-0 2017-06-14T00:00:00.000-0800 I NETWORK [ReplicaSetMonitor-TaskExecutor-0]
      All nodes for set Cluster0-shard-0 are unreachable".
