An upgrade to Mongodb 4.2 can introduce mixed version entries in unique indexes due to the unique index format change in 4.2. Update major_version_upgrade.js to create unique index entries using 4.0 binary followed by inserts using the 4.2 binary. Check that index is readable with mixed version entries.
