The storage catalog should know nothing about the MultiIndexBlock class that builds indexes using the catalog interface

      We use the Collection and Index catalogs for index building. Right now the index building code, MultiIndexBlock, is included in the catalog_impl library, and Collection handles creation. MultiIndexBlock should be separated from the catalog, to use the catalog instead of be a part of it. The catalog should know nothing about MultiIndexBlock.
