We have some code to manage a pointer to an "index head", which is the start of a linked list of index catalog entries for MMAPv1. However, with the removal of that storage engine this field is now unused. For backward compatibility with 4.0 and earlier versions, still store a "0" in the persisted BSON catalog entry.
