IndexCatalog should add the 'ns' field to the index spec if it's missing

      With the removal of the 'ns' field from index specs in v4.4, we need to ensure that in v4.2 we add the 'ns' field to the index spec explicitly if it's missing. This could happen when running with mixed version nodes where at least one node is v4.4.
