Make the IndexCollectionEntryContainer thread-safe

      In preparation for concurrent creation of (empty) indexes, make the IndexCollectionEntryContainer thread-safe. That is: use a mutex to synchronize access to the underlying data-structures.

      IndexIterators must remain valid when adding new indexes to the container.
      Include unit tests that check the safety and invalidation properties.
