two phase index builds hangs in flow control ticket test suites with low ticket limit

      When two phase index builds were turned on by default, we started observing timeouts in the flow control test suites where the ticket limits were set to a very low value (single digits). This ticket captures the work necessary to re-enable two phase index builds in these tests.
