Remove v4.4 FCV handling for collection and index catalog entry 'ns' field

      Rename collection handles index specs and handles 'ns' field only for FCV 4.2 and lower level in the catalog entry bson as well, added in SERVER-44247.

      Durable catalog added in SERVER-41697

      Looks like jstests/multiVersion/remove_ns_field_in_index_spec.js can be removed.

      Please check the SERVER ticket(s) that added the FCV gates for anything else incidental to remove, particularly any testing that can be deleted.
