Log the application client starting or stopping the balancer

      This improvement builds on top of SERVER-37422, which logs balancer start/stop commands in config.actionlog along with the mongoS the request was routed through (clientAddr).
       
      This ticket is to log the remote (application) client that issued those commands. This is useful in the field to identify the source of an unexpected balancer status.
