      We observed a consistent behavior when the error
      "Unable to reach primary for set "...
      in the RSM method Refresher::getNextStep() is printed it is not trying all hosts again. More targeted logging around replenishment of the _scan->hostsToScan is needed. This is in branch 4.0.
