capped collection _id index creation on secondaries not triggered by updates

      Capped collections are supposed to automatically get an _id index on secondaries. However, the creation of this _id index is triggered RARELY by some number of inserts. Doing a bunch of updates without inserts won't trigger the index creation. I'm uploading a js test which illustrates this.
