GDB_INDEX and SPLIT_DWARF enabled together cause issues.

      In a DWARF_VERSION=4 build, GDB_INDEX and SPLIT_DWARF enabled cause seg faults for gdb. We should default SPLIT_DWARF off if GDB_INDEX is on and if both are requested on the command line, produce an error.
