Ensure that we respect the internalQueryExplainSizeThresholdBytes param when creating explain

Also investigate whether we need to explicitly respect the maxBSONDepth parameter (it might be that we would always hit the internalQueryExplainSizeThresholdBytes first). It doesn't seem like classic explain references this.
