Remove support for pre-4.2 Server versions and dump files

      We have removed a few flags and features related to pre-4.2 Server versions, as well as cross-version metadata dump/restore. We only support data movement across versions, not metadata. These are:

      * mongorestore does not pass {{ignoreUnknownIndexOptions}} when creating indexes. Unsupported index options are an error.
      * mongorestore not special case {{autoIndexId}} in index options. If this exists and is set to {{false}}, it will cause an error.
      * mongorestore no longer removes the {{noPadding}} and {{usePowerOf2Sizes}} fields when running a {{collMod}} command as part of oplog restore.
      * mongorestore no longer supports indexes stored in a {{system.indexes}} collection. This was how indexes worked before Server version 3.0.
      * The mongorestore {{--convertLegacyIndexes}} and {{--fixDottedHashIndex}} options have been removed.

      We do not test with any pre-4.2 Server versions at all, and have not done so for over a year.

      Our public documentation also says that we do not support cross-FCV version dump/restore except for data BSON files. All of the code for handling pre-4.2 metadata contradicts this.
