This is a general cleanup ticket, we currently need a hole list to copy log files across and don't use __wt_copy_and_sync. This is because copy and sync doesn't honour layering, I think, and it also doesn't take a read_size param.

By removing the dependency on holes we can simplify the log transfer path and the file setup path significantly.
