I added a "cache_resident" configuration flag, that turns off page eviction and quits maintaining hazard references. It gives me almost 15% performance improvement on pixiebob in simple, in-memory tests, seems useful for highly contentious objects that are relatively small compared to the memory available.

I've put a couple of comments in the diffs where a change wasn't obvious.
