Michael, this passes tests and I've updated all of the documentation.

It's probably worth doing a fast review of session/session_api.c, specifically:

1. I've disallowed session.reconfigure if there are transactions running,
2. session.txn_begin resets cursors – I doubt that's necessary, but it seemed safer.
