Add a fault point to evict a page when a cursor stops referencing it. Write a test to drive the change. Consider what configuration options would make sense. Is the fault going to be turned off after hitting once, or do we want a series of pages to be evicted as the cursor moves along? Do we want to evict all the places the fault hits or do we want to do with a certain probability. Do we need one fail point or multiple such fail points, etc.

Also, consider what cursor operation(s) we want the fault to be associated with.
