Remove LSM references from tiered cursor code

A large portion of the tiered cursor code was copied from the LSM cursor code. As such, it still contains a lot of references to LSM-isms in the comments and variable names.  To avoid future confusions, this needs to be changed to use Tiered Storage terminology.
